Metric vs US crochet hook sizes?

I came across a bunch of craft books at a garage sale recently and want to try out some crochet patterns, but they have crochet hooks with letter sizes and the crochet hooks in the craft stores here are in mm.

I did some googling and I managed to find a conversion chart, but it doesn't list the bigger ones. Can anyone fill in the blanks?
