Stencil & paint safety?

craftgrrlThinknig about making stencils and using them on various articles of clothing and fabrics. Would it be safe to use spray can paint on underwear (the top front part)? The fabric is a cotton material. Would it be safer to wash it (after painting), before use? I guess these are silly questions, but I just want to know if anyone has any help on this?
